- What You'll Do

As a Skilled Home Health LPN, you'll provide high-quality nursing care to patients in their homes under the direction of an RN or physician. You'll play a key role in helping patients stay safe, comfortable, and out of the hospital.

* Deliver skilled nursing care according to the plan of care and physician orders

* Support patients and caregivers with education and self-care techniques

* Observe, assess, and communicate changes in patient condition to the RN Case Manager

* Document care accurately and timely in the EMR

* Promote patient safety and help prevent avoidable hospitalizations

* Build trusting relationships with patients, families, and care team members

- What We're Looking For

* Graduate of an accredited nursing program

* Active LPN license in the state

* At least 1 year of clinical experience (home health or hospice experience preferred)

* Comfortable working independently in the field

* Valid driver's license, reliable transportation, and ability to travel within Elkhart & LaGrange Counties

* Ability to meet physical demands of the role

* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
