- Your Role at a Glance

You'll serve as the primary nurse for an assigned caseload, delivering high-quality care across the home health continuum by:

* Completing start-of-care, routine, and follow-up nursing visits

* Performing comprehensive patient assessments

* Developing and updating individualized plans of care

* Monitoring patient progress and identifying changes in condition

* Communicating with physicians, clinical leaders, and care teams

* Documenting care accurately and timely in the EMR

* Ensuring compliance with state, federal, and agency guidelines

* Maintaining patient privacy and HIPAA compliance

- What We're Looking For

* Active, unrestricted RN license

* At least 1 year of nursing experience in a clinical setting

* Associate's or Bachelor's Degree in Nursing preferred

* Home Health or Hospice experience preferred

* Experience with Home Care Home Base a plus

* Reliable transportation, valid driver's license, and auto insurance

* Ability to travel within the service area (approximately 50%)

* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
