On an Average Day You Will:

(includes but not limited to)

* Provide skilled nursing care under the direction of an RN or physician

* Follow and verify the Plan of Care at each visit

* Communicate changes in condition to the RN Case Manager and physician

* Document care, medications, and patient progress accurately

* Educate patients and families on self care and safety

* Help prevent avoidable hospitalizations and ER visits

What You Bring:

* Graduate of an accredited nursing program

* Active Indiana LPN license

* At least one year of clinical experience

* Home health or hospice experience preferred

* Valid driver's license, insurance, and reliable transportation

* Ability to lift, push, pull, or support up to 50 pounds
