This is a full‑time, hybrid position located in the Elkhart or Goshen, IN area. You must travel 60% of the time for in‑person patient appointments. The Field Case Manager works closely with treating physicians/providers, employers, customers, legal representatives, and injured/disabled persons to create and implement a treatment plan that returns the injured/disabled person to work appropriately and ensures cost‑effective healthcare services.
Responsibilities- Demonstrate knowledge and competency in case management standards of practice.
- Use advanced knowledge of injury types, medications, comorbidities, treatment options and job duties to advise on a treatment plan.
- Interview disabled persons to assess overall recovery, including whether injuries or conditions are occupational or non‑occupational.
- Collaborate with treating physicians/providers to create and implement individualized treatment plans.
- Work with employers and physicians to modify job duties where practical to facilitate early return to work.
- Evaluate and modify case goals based on injured/disabled person’s improvement and treatment effectiveness.
- Independently manage workload, prioritizing cases and deciding best management.
- Attend injured worker appointments when appropriate and prepare status updates for customers.
- Education:
Associates Degree or Bachelor’s Degree in Nursing or related field. - Experience:
2+ years clinical practice preferred; workers’ compensation–related experience preferred. - Skills:
Advocate recommendations effectively with physicians, employers, and customers; work independently; basic computer skills including Excel, Word, Outlook; proficient written communication. - Certifications & Licenses:
Active Registered Nurse (RN) license in good standing; URAC‑recognized case management certification (CCM, CDMS, CRC, CRRN, COHN, RN‑BC, ACM, CMAC, CMC). - Travel:
Reliable transportation and ability to travel to in‑person appointments in assigned geography. - Internet:
Reliable internet access.

The expected base pay ranges from $75,600 – $80,000 annually; in addition to base salary you may participate in a productivity‑based bonus program.
