Outpatient Nurse (LPN / RN) – Elkhart
Oaklawn Psychiatric Center seeks an outpatient psychiatric nurse to play a central role in helping clients achieve recovery and improve overall wellbeing. Candidates will work across our campuses in Elkhart, Goshen, and South Bend, providing direct patient care and supporting a multidisciplinary team.
OverviewAs an Outpatient Nurse, you will assess clients, provide health education, administer medications, and coordinate care with psychiatrists, therapists, and case managers. Your work will support a diverse population of children, adolescents, and adults dealing with mental health or substance use issues.
Benefits- Competitive compensation:
Minimum $28.32/hr for LPN or $30.30/hr for RN - Employment bonus: $3,000 for FT LPN or $4,000 for FT RN (pro‑rated by FTE)

- Collect and document data regarding clients’ mental and physical health status
- Assess clients prior to face‑to‑face sessions with the psychiatrist or NP
- Provide health education, somatic therapies, medication management, and crisis intervention
- Respond to medication calls and administer medicines per protocol
- Report adverse drug reactions to physician and pharmacist, and manage medication storage
- Conduct interviews and complete Medicaid and SSI Disability evaluations efficiently
- Maintain accurate and timely patient charts in the electronic health record
- Participate in care coordination with external providers to facilitate access to healthcare and resources
- Current Indiana RN or LPN license
- At least 2 years of post‑degree psychiatric nursing experience (preferred)
- Strong communication and therapeutic relationship skills
- Knowledge of medication safety protocols and documentation standards
- Ability to work independently and as part of a multidisciplinary team
Primary office hours:
Monday–Friday, 8 a.m.–5 p.m. with optional evening work as needed.
