Position: Registered Nurse (RN) - Hospice

Responsibilities
  • Provide care and services to a group of patients by visiting them at home, skilled nursing facilities, and assisted living facilities.
  • Act as a contributing member of the interdisciplinary team, providing expertise in psychosocial assessments, interventions, and community resources.
  • Work in case management, admissions, or resource roles as needed.
  • Work independently or as part of a team of nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Prioritize and triage multiple concerns, manage visits, documentation, and meetings.
  • Learn and use the hospice electronic medical records system.
  • Drive reliably within the service area, holding a valid driver’s license and auto insurance.
Qualifications
  • Current Registered Nurse (RN) licensure in the state(s) of practice, in good standing.
  • Strong teamwork skills, able to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Experience working one‑on‑one with patients and receiving support from a patient‑care team.
  • Independent work self‑management with schedule, phone, email, and written communication.
  • Effective time‑management skills and ability to respond to urgent needs.
  • Ability to learn a new electronic medical records system.
