About the role
At Brighton/Suncrest Hospice, we aim to change the expectations of hospice care in our area by providing exceptional care and service to our patients. We are proud to be a CHAP certified hospice and are committed to increasing staff-to-patient ratios to provide more clinical visits while lowering clinician caseloads.

- Provide care and services to a group of patients by visiting them at home, skilled nursing facilities, and assisted living facilities.
- Act as a contributing member of the interdisciplinary team, providing expertise in psychosocial assessments, interventions, and community resources.
- Work in case management, admissions, or resource roles as needed.
- Work independently or as part of a team of nurses, social workers, chaplains, volunteers, and CNA leadership.
- Prioritize and triage multiple concerns, manage visits, documentation, and meetings.
- Learn and use the hospice electronic medical records system.
- Drive reliably within the service area, holding a valid driver’s license and auto insurance.
- Current Registered Nurse (RN) licensure in the state(s) of practice, in good standing.
- Strong teamwork skills, able to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Experience working one‑on‑one with patients and receiving support from a patient‑care team.
- Independent work self‑management with schedule, phone, email, and written communication.
- Effective time‑management skills and ability to respond to urgent needs.
- Ability to learn a new electronic medical records system.
