Position
Part Time/Weekend Physical Therapist Case Manager
Coverage Area Elk Grove
What you’ll do as a Home Health Physical Therapist Case Manager- Perform an evaluation in the home, develop a care plan, and work with Physical Therapy Assistants (PTAs) to follow the plan.
- Help patients with mobility, movement, and regain independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patients’ progress to the MD.
- Provide skilled physical therapy services in patients’ homes in partnership with other clinicians.
- Supervise PTAs per agency and regulatory standards where applicable.
- Maintain thorough documentation and patient records in accordance with regulatory requirements.
- Current unrestricted CA Physical Therapist License
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge of local community resources
- Understanding of development, behavior, and the influence of culture on health care
- Knowledge of state and federal homecare regulatory guidelines (preferred)
- Experience in home health care (highly preferred)
- Experience with high acuity patients (a plus)
- Experience working with an interdisciplinary team (highly desired)
- Case Management fundamentals (highly desired)
- Self-directed, empathetic, critical thinker with outstanding interpersonal skills
- Leadership and organizational abilities
$120,000 - $135,000 per year
Wage details:
Physical Therapist, Level 1: 120,000 – 135,000 plus a quarterly incentive; starting pay commensurate with experience. 21
