Position
Part Time/Weekend Physical Therapist Case Manager

Coverage Area Elk Grove

• Perform an evaluation in the home, develop a care plan, and work with Physical Therapy Assistants (PTAs) to follow the plan.
• Help patients with mobility, movement, and regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to the MD.

• Provide skilled physical therapy services in patients’ homes in partnership with other clinicians.
• Supervise PTAs per agency and regulatory standards where applicable.
• Maintain thorough documentation and patient records in accordance with regulatory requirements.

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
• Understanding of development, behavior, and the influence of culture on health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care (highly preferred)
• Experience with high acuity patients (a plus)
• Experience working with an interdisciplinary team (highly desired)
• Case Management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker with outstanding interpersonal skills
• Leadership and organizational abilities

$120,000 - $135,000 per year

Wage details:
Physical Therapist, Level 1: 120,000 – 135,000 plus a quarterly incentive; starting pay commensurate with experience. 21
