Overview
Welbe Health PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model. Our Home Care Assistants are a vital part of our team, providing aid to participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT).
Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to Welbe Health participants in their home, as scheduled and directed.
- Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including bathing, dressing, grooming, toileting, and lifting when transferring participants
- Assist participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and light meal preparation
- Measure and document participants’ vital signs as instructed
- Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes
- Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed
- Provide translation services as needed
- Work a stable, full-time schedule with participants assigned to fill a full workday; re-assigned in cases of cancellations or unforeseen circumstances
- Visit 2-5 patients per day depending on participant needs and care plans, with shorter individual shifts that add up to an 8-hour workday
- Receive reimbursement for travel time and mileage between participants
- Salary/Wage base range for this role is $21.00 - $25.22 hourly + Bonus

- Actual pay will be adjusted based on experience and other qualifications
- High school diploma or equivalency
- Current Basic Life Support (BLS)/First Aid Certification required
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
- Bilingual preferred
- Must be registered with Home Care Registry prior to start date
