• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA or its equivalent
• Licensed or registered to practice as a PT in the state of practice
• Preferably at least two (2) years of experience supervising a Physical Therapist Assistant in Home Health
• Extensive and diverse training in orthopedic, neurology, cardio‑pulmonary, and related areas
• Ability to read, follow written instructions, and document care given
• Self‑directing and able to work with little direct supervision
• Current CPR certification
• Flexible, cooperative, and demonstrates tact, patience, and good personal hygiene
• Licensed driver with an insured automobile in accordance with Lorian Health requirements
• Ability to verbally communicate in the client’s language and document all client‑related activity practices

• Provide treatment as ordered by attending physician, dentist, or podiatrist
• Assist in developing and updating the plan of treatment or Plan of Care (POC) in an electronic medical record system
• Supervise 1–2 Physical Therapy Assistants, ensuring productivity standards are met
• Evaluate patients’ level of function and report findings to the attending physician and case manager
• Advising, consulting, and instructing family members and Lorian personnel on patients’ therapy program
• Participate in in‑service education programs to enhance professional development

• Pay‑per‑visit model: $70–$140 per visit, commensurate with experience and visit type
