Job Scope

In collaboration with agency psychiatrists and the medical secretary, the Adult Therapist/Medical Case Manager will deliver reassessment and treatment planning services, brief individual crisis intervention or supportive counseling, symptom‑management skill‑building groups, and case management or community support to adult clients receiving medication-only services. The therapist provides individual, group, and family treatment that targets functional improvement.

Primary Responsibilities
  • Provide initial intake screening and mental‑health assessments that identify service needs and recommend care options
  • Share assessment findings with clients
  • Involve the client (or couple) in developing treatment‑plan goals, objectives, and interventions that address their behavioral health needs
  • Deliver face‑to‑face mental‑health reassessments as needed, at least annually for medication‑only clients
  • Meet with medication‑only clients for treatment‑planning review and modification at least every 6 months
  • Provide brief crisis intervention, stabilization, and supportive counseling to medical‑only clients
  • Consult regularly with psychiatrists on specific client‑related clinical issues
  • Offer individual, marital, group, or family therapy services
  • Provide case‑management services to facilitate access to benefits and services for clients and their families
  • Coordinate care by initiating or participating in client‑centered consultations with internal and external providers
  • Reassess and review treatment progress with clients and families at prescribed intervals
  • Provide backup coverage for the Medical Secretary
  • Assist in establishing effective communication between the Center, other social agencies serving clients, and the community
Performance Requirements
  • Work cooperatively with Adult Services team members to ensure timely client access to quality services
  • Meet agency quality and compliance standards by accurately completing clinical documentation and service reporting promptly
  • Meet or exceed service productivity targets
  • Implement evidence‑based practices to ensure effective and efficient treatment that best addresses client needs
  • Participate in staff training and development seminars
  • Work cooperatively with other Center teams to maintain smooth continuity of care when cases share across teams
  • Become familiar with and utilize outside resources as needed for comprehensive client services
  • Participate in supervision with the Adult Outpatient Services Coordinator and in regular performance appraisals as directed
  • Understand the different funding sources available to the population served and the billing requirements for each source
Education and Experience
  • Master's degree from an accredited college or university in Social Work, Psychology, or a closely related discipline, and be licensed or eligible for certification
  • Supervised experience in providing counseling and/or psychotherapy services
  • Knowledge of diagnostic assessment, long- and short-term treatment, family systems theory, and group treatment
  • Basic computer and keyboarding skills
Schedule/Timekeeping
  • Full‑time, exempt, 37.5‑hour work week
  • Prepared to work a flexible schedule, including evenings and Saturdays, to meet the needs of the target population
