This is a part-time on-site role for an Advanced Practice Nurse located in Elizabethtown, KY. The Advanced Practice Nurse will be responsible for providing high-quality patient care, including assessments, diagnosis, and treatment planning. They will collaborate with physicians and healthcare staff, ensure accurate documentation, and monitor patient outcomes. The role also involves engaging with patients and families to provide education and address concerns, and staying current with best practices and industry standards.

• Clinical skills in patient assessment, diagnosis, and creating treatment plans
• Experience in post-acute or primary care settings, with skills in monitoring patient outcomes
• Strong communication, collaboration, and patient education skills
• Proficiency in using electronic health records and maintaining accurate documentation
• Active Advanced Practice Nurse (APRN) license in the state of Kentucky
• Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) is preferred
• Strong organizational and time-management skills with the ability to work independently
