Position: Licensed Clinical Social Worker (LCSW)

Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

We are seeking a Licensed Clinical Social Worker (LCSW) or a Licensed Independent Social Worker (LISW) to join our team! You will provide recovery-oriented rehabilitation services in group or individual settings. Our care focuses on individuals in long-term care settings, including skilled nursing, assisted living, or personal care. We address a wide range of issues from anxiety and depression to severe mental illnesses and end-of-life concerns.

As part of a multidisciplinary team, our clinicians have the opportunity to significantly improve residents' quality of life. We aim to solve problems, deliver excellent clinical care, and serve as a crucial link in the care of this vulnerable population.

Essential Duties and Responsibilities:

  • Assess each resident with valid consent to determine individual needs. Develop and follow a treatment plan to meet these needs, ensuring all treatment is medically necessary.
  • Perform written or verbal consultations with clients' primary care physicians, family, facility staff, and Eventus Whole Health support staff as needed.
  • Document treatment plans, psychotherapy sessions, and updates according to Eventus Whole Health’s policies, providing written documentation within 48 hours for inclusion in the resident’s medical record.
  • Meet with the Resident Care Coordinator or designated staff upon arrival and departure to review care needs and communicate findings or concerns.
  • Establish and maintain open, positive communication with facility staff and administration. Provide verbal and/or written feedback regarding treatment plans and relevant caregiver information.
  • Adhere to Eventus Whole Health standards, policies, procedures, and all legal, regulatory, and ethical guidelines governing healthcare practice. Comply with all regulatory agencies.
  • Conduct oneself professionally at all times.
  • Maintain patient confidentiality, including proper use of communication tools and records, and comply with HIPAA regulations.
  • Consult with the clinical supervisor when facing unfamiliar residents, family requests, or care recommendations.
  • Qualifications/

    Education:

    • Psychologists:
      Doctoral degree from an accredited institution, with a current license as a doctoral-level Psychologist and Health Services Provider in the practicing state.
    • Licensed Clinical Social Workers:
      Master’s degree in social work and a current license to practice in the state.

    Knowledge:

    • Assessment and psychotherapy techniques, population-specific interventions, and treatment plan delivery.
    • Use of diagnostic tools, including the DSM.
    • Rules and regulations governing psychotherapy practice.
    • Organization’s policies and procedures.

    Skills Required:

    • Gathering and analyzing physiological, socioeconomic, behavioral, and emotional patient data.
    • Evaluating patient problems and providing appropriate advice, intervention, or referral.
    • Effective written and verbal communication.
    • Exercise of initiative, judgment, and discretion.
    • Establishing and maintaining effective working relationships.

    Abilities:

    • React calmly and effectively in emergencies.
    • Work collaboratively with healthcare team members.
    • Make responsible decisions within scope of practice.
    • Evaluate and recommend quality improvements.
    • Handle confidential and sensitive information appropriately.
