Job Summary: The Behavioral Health Specialist provides care‑management services and evidence‑based short‑term psychotherapy to patients with behavioral health needs. The role is a key resource for Care Teams across Oak Street Health primary clinics, coordinating and supporting behavioral healthcare in collaboration with Care Teams and a psychiatric consultant.
Core Responsibilities- Collaborate with primary care teams to identify appropriate patients for the Behavioral Health Program.
- Screen and assess patients for common mental health and substance use disorders.
- Develop care plans and facilitate referrals to Oak Street’s psychiatric consultant or external resources.
- Provide short‑term counseling and evidence‑based treatments.
- Coordinate care with the psychiatric consultant, including warm handoffs for telehealth visits and collaboration on care plans.
- Establish relationships with primary care teams and consistently communicate changes in behavioral health care plans.
- Maintain accurate and timely documentation of patient encounters and counseling sessions in Oak Street’s electronic medical record.
- Participate in regional and organizational efforts to advance the Behavioral Health Program through workshops, feedback sessions, and surveys.
- Possibly cover multiple clinics, depending on clinic maturity and panel size.
- Other duties, as assigned.
- Master’s Degree in a mental health counseling discipline (Social Work, Counseling, Marriage and Family Therapy, etc.) from an accredited school.
- Highest clinical level license (or local equivalent) in the state of employment.
- Experience with screening for common mental health and/or substance use disorders.
- Experience with assessment and treatment planning for common mental health and/or substance use disorders.
- Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral).
- US work authorization.
- Proficiency in Spanish; bilingual spoken and written proficiency required.
- Proficiency in providing brief, structured intervention techniques (Motivational Interviewing, Behavioral Activation, Cognitive Behavioral).
- Working knowledge of differential diagnoses of common mental health and/or substance use disorders.
- Proficient PC skills.

Pay Range: $66,575.00 – $ (base, varies by experience, education, geography)
