Acceptable Experience and Education
Bachelor’s degree from an accredited college or university with a major in social, behavioral or human services related field, and one (1) year experience of case management or care coordination experience. Experience working with juveniles is preferred.
Bilingual (Spanish/English) preferred.
Or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.
Minimum Requirements:Knowledge, Skills, and Abilities
Knowledge of: case plans aligned to treatment recommendations; services and community resources that may be utilized to facilitate the rehabilitative and reintegration process; crisis stabilization techniques and/or resources; socialization services; coping services; self-regulating; life skills and independent living groups and/or services;
Departmental, TJJD, THHSC and licensing standards;
English usage, grammar, punctuation and spelling; techniques for effective interpersonal communications; modern office procedures, methods and computer equipment to include Windows and Microsoft based products.
Skill/Ability to: learn and apply current juvenile justice theories and practices and evidence-based programs; practices relevant to case management practices for individuals with a behavioral and/or mental health disorder; be willing to work in a pre and post adjudication secure and none-secure residential setting; possess a high degree of maturity coupled with an interest in working with children/adolescents, their families and ability to work positively and effectively with maladjusted youth;
develop case plans aligned to treatment recommendations; identify services and community resources that may be utilized to facilitate the rehabilitative and reintegration process; aid in the amelioration of identified symptoms/behaviors associated with a behavioral health and/or mental health disorder; apply crisis stabilization techniques and/or resources; facilitate socialization, coping, self-regulating, life skills and independent living groups and/or services; adhere to Departmental, TJJD, THHSC and licensing standards;
possess excellent communication and interpersonal skills; engage youth and family members in the therapeutic and rehabilitative processes; foster teamwork and partnerships with co-workers, departmental personnel and community stakeholders; work positively and effectively with all types of children including severely maladjusted youth; possess organizational and time management skills; record keep; ensure that services are prioritized; adhere to Departmental, State and Federal standards and laws regarding treatment records;
work efficiently both independently and as part of a team; establish and maintain effective working relationships with departmental personnel, clients, other County employees and officials, clinical service providers, law enforcement, the courts and the general public; flexible to the needs of the department and program and make required changes to fulfill those needs; successfully complete the department’s pre-employment requirements; pass a Criminal Background Check;

Provides service referrals and linkages to the community to assist with the needs of the family while youth is in placement and upon reintegration into the home or while on community based supervision;
Coordinates and/or attends home visits, phone calls, conference calls, Admission, Review, Dismissal( ARD), school meetings, facility meetings, case staffings, court hearings;
