Seeking experienced RN Case Managers to coordinate care for service members processing through mobilization and demobilization. Play a critical role in ensuring soldiers receive appropriate medical care and follow-up.

Responsibilities
  • Manage cases for service members with identified medical/behavioral health issues
  • Coordinate care between multiple providers and specialties
  • Interview service members and review comprehensive medical records
  • Implement and monitor plans of care
  • Maintain daily unit trackers
  • Communicate with commanders and medical staff
  • Document all case management activities in MHS Genesis
  • See 10-30 service members weekly
Qualifications Required
  • Degree: Associate's or Bachelor's in Nursing from ACEN or CCNE-accredited program
  • License: Current RN license (any U.S. state)
  • Certification: One of the following:
    • CCM (Certified Case Manager)
    • RN-NCM (Nurse Case Manager)
    • CMC (Care Manager Certified)
  • Additional: Current BLS certification
  • Experience: 1+ year as full-time RN Case Manager within last 2 years
  • Citizenship: U.S. Citizen
