We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. This role requires clinical expertise, sound judgment, and the ability to assess patient needs while working closely with physicians to develop and carry out individualized plans of care.

• Conduct in-home patient visits, including skilled assessments, admissions, recertifications, discharges, transfers, and treatments.
• Complete OASIS assessments in compliance with Medicare requirements.
• Develop nursing care plans aligned with the physician’s plan of care.
• Monitor patient progress, condition, and compliance; update care plans as needed.
• Communicate regularly with patients, physicians, and care teams regarding treatment response, changes in condition, and care needs.
• Coordinate referrals for additional services (therapy, social work, medical equipment, etc.).
• Supervise LVNs, Home Health Aides, contract staff, and other care providers.
• Provide orientation for HHAs and support other agency programs as needed.
• Participate in the on-call rotation.
• Support Quality Assurance/Quality Improvement activities and ongoing training.

• Graduate of an accredited school of nursing.
• Current RN license in Texas (or multi-state NLC license if applicable).
• Minimum of 1 year RN experience preferred.
• Strong knowledge of nursing theory, clinical practice, and Medicare/Medicaid requirements (OASIS experience a plus).
• Ability to work independently, exercise sound clinical judgment, and problem-solve effectively.
• Excellent written and verbal communication skills (Spanish fluency a plus).
• Proficient with point-of-care software.
• Bring your own device – this Per Visit position is BYOD. Please ensure you have a cell phone, tablet, or laptop that you can perform required documentation on. The office will have a tablet that can be checkout out and checked back in when doing visits, if you do not have a device, but availability is not always guaranteed due to the need of other per visits nurses.

• Ability to travel independently to patient homes.
• Ability to lift/carry medical supplies and assist patients safely with mobility and care needs.
• Manual dexterity to perform procedures such as IV placements and blood draws.
• Flat rate of pay – The Per Visit rate factors in driving to make each visit. Additional mileage pay is not included when doing Per Visit work.
