Overview
Job Title: Registered Nurse Case Manager.
Location: Soldier Readiness Processing Center (SRP), William Beaumont Army Medical Center (WBAMC), 18511 Highlander Medics St, Fort Bliss, TX 79906, and outlying clinics.

Required:
3 FTEs.
Work Schedule: Monday-Friday, (SRP operational hours). Up to 48 mission-driven overage hours per FTE may be required annually beyond regularly scheduled duty hours, as authorized by the Government.
Qualifications- Education:
Graduate of an accredited Registered Nurse (RN) program (Associate Degree in Nursing or Bachelor of Science in Nursing). - Experience:
- Minimum of one (1) year of clinical nursing experience within the last two (2) years performing case management, care coordination, or utilization management functions.
- Experience reviewing medical records and coordinating care for patients with complex medical conditions.
- Experience collaborating with providers, behavioral health, pharmacies, and ancillary services to resolve care barriers.
- Experience managing referrals, follow-up appointments, and continuity of care across multiple services.
- License / Certification:
- Current, unrestricted Registered Nurse (RN) license issued by any U.S. state or territory.
- Basic Life Support (BLS) certification (AHA or Red Cross) required.
- Other Requirements: U.S. Citizenship required.
- Perform medical case management in support of Soldier Readiness Processing (SRP), mobilization, demobilization, redeployment, and separation activities.
- Review medical records, readiness assessments, profiles, and provider documentation to identify service members requiring follow-up care, referrals, or additional coordination.
- Coordinate medical care, specialty referrals, diagnostics, and follow-up appointments to support readiness and disposition decisions.
- Track service members with medical conditions that impact deploy ability and ensure required evaluations, waivers, or follow-up actions are completed.
- Communicate with physicians, nurse practitioners, physician assistants, pharmacists, and behavioral health providers to resolve care gaps and support clinical decision-making.
- Monitor progression of cases to ensure timely completion of required medical actions prior to deployment, redeployment, or separation.
- Document case management activities, coordination efforts, and disposition outcomes in MHS GENESIS, ensuring accuracy and timeliness.
- Maintain workload and case volume comparable to other RN Case Managers performing similar SRP functions.
- Support surge operations during high-volume MOB/DEMOB cycles, including extended duty hours when required.
