Bienvivir All-Inclusive Senior Health ("Bienvivir") is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly ("PACE").

PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position:

WOUND CARE RN

The Wound Care Registered Nurse, under the supervision of the Director of Nursing, is responsible for overseeing Bienvivir’s wound care program. This role provides leadership and direction to LVNs in the treatment and management of wounds, ensuring evidence-based and participant-centered care. Key responsibilities include coordinating and tracking wound treatments, supervising data collection for quality improvement initiatives, and guiding staff in wound prevention and education.

The Wound Care RN participates in care plan development, collaborates with the Primary Care Providers and the Interdisciplinary Team (IDT) in the care of participants with wounds, and supports data-driven decision-making to improve clinical outcomes.

RESPONSIBILITIES
  • Oversee the wound care program, including coordinating and monitoring wound care treatments provided by LVNs for all participants enrolled in Bienvivir.
  • Supervise and support LVNs in the delivery of wound care, ensuring adherence to established protocols and standards of care.
  • Provide direct wound care as needed, including making visits to participants’ places of residence.
  • Ensure timely and effective wound care interventions through ongoing monitoring and coordination.
  • Track wound progress and maintain accurate documentation in compliance with regulatory standards.
  • Oversee data collection and reporting related to wound care for Quality Improvement initiatives.
  • Lead the Wound Care Committee and actively participate in the Quality Improvement Committee and other committees as needed.
  • Provide staff education on wound prevention, treatment protocols, and best practices.
  • Support staff development by conducting general in-services, instruction, and orientation to the wound care program.
  • Guide staff in obtaining, analyzing, and applying wound care data to support effective clinical decision making.
  • Participate in individualized care plan development for participants with wounds.
  • Collaborate with the Interdisciplinary Team (IDT) to ensure coordinated, participant-centered care.
  • Collaborate and maintain ongoing communication with Primary Care Providers (PCPs) regarding assigned participants, ensuring they are kept informed of wound status, treatment progress, and any changes in condition. Verify that provider orders are obtained, documented, and carried out as directed.
  • Assist in providing wound care.
  • Evaluate the effectiveness of wound care strategies in relation to participant responses and expected outcomes.
  • Other duties as assigned by the supervisor.
  • QUALIFICATIONS / REQUIREMENTS
  • Graduate of an accredited nursing program with a current license to practice as a Registered Nurse in the State of Texas.
  • Valid wound care certification preferred.
  • Current CPR certification is required.
  • Minimum of two years of wound care experience required.
  • Minimum of one year of experience with a frail or elderly population preferred.
  • Valid Texas driver’s license and insurance.
  • Bilingual (English/Spanish) preferred.
