Job Summary
Navigates, coordinates and assists patients requiring clinic and community resources in the Neighborhood Health Care Centers (NHC) in conducting population health management to meet patient care needs. Assumes responsibility for coordinating resources and navigating within the clinics to meet patient requirements in accordance to regulatory standards The Joint Commission Requirements/ Primary Care Medical Home models and the Nursing Practice Act.
Skills- Possesses effective oral and written communication skills.
- Bilingual highly preferred (English/Spanish).
- Effective patient interviewing and counseling techniques.
- Ability to read and comprehend English at a level necessary to follow written and oral instruction and safety precautions.
- Ability to utilize computers for documentation purposes.
- Ability to negotiate and resolve conflict.
- Willingness to accept and manage change.
- Ability to analyze situations and solve problems.
- Skilled in the use of computer systems and related software applications.
Three years of experience as a Registered Nurse required, preferably in Emergency Room, Urgent Care, Critical Care, or Clinic setting.
License and CertificationsCurrent and active license to practice as a Registered Nurse in the State of Texas is required. Current CPR training required.
Education and TrainingBachelor degree in Nursing is required.
