General DescriptionThe purpose of this job is to provide moderately complex case management work. Work involves developing and maintaining long‑term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Specific duties and responsibilities may vary dependent upon the functional area assigned. This class works under close general supervision according to set procedures but determines how or when to complete tasks.
Dutiesand Responsibilities
- Provides ongoing case management services, skills training and rehabilitation services for youth and/or adults with mental illness and/or individuals with developmental disabilities.
- Provides rehabilitative services including general symptom management, community living skills, and employment related skills to increase client independence and maintain community tenure.
- Provides 24‑hour on‑call support, psychosocial rehabilitation three to five times per week, mental health decompensation support, safety plan, crisis intervention, medication support, and collaborates with inpatient hospitals.
- Collaborates with psychiatrist to formulate personalized treatment plans to fit individual needs. Medication management includes compliance with medication monitoring, obtaining injections, and providing transportation.
- Assists with improving living skills, finding appropriate housing, moving belongings, setting up utilities, budgeting and payments, and food. Coordinates representative payee services.
- Maintains compliance with all Joint Commission training requirements relevant to job duties, including those dealing with the use of restraints and physical holding of individuals receiving services.
- Performs other duties as assigned.
Requires a Master’s degree in Psychology, Social Sciences or a related behavioral field, supplemented by two (2) years of direct casework/case management experience; or possession of any equivalent combination of education, training, and experience that provides the requisite knowledge, skills, and abilities.
Experience with trauma‑informed services; cognitive behavioral therapies, including Dialectical Behavioral Therapy (DBT); and motivational therapies including the use of incentives, preferred.
Required Knowledge and AbilitiesKnowledge of trauma‑informed theories, principals, and practices (includes multi‑faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions), preferred.
Physical DemandsPerforms sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

- Must be able to be credentialed as a Qualified Mental Health Practitioner Community Service (QMHP‑CS) within the timeframe designated by EHN.
- Must possess and maintain a valid state Driver's License with an acceptable driving record.
- Must be able to pass a TB, criminal background and drug screen.
