Amerita, Inc. is a leading provider of home infusion therapy. We are seeking a Pharmacist to join our pharmacy team and help us grow to become one of the top home infusion providers in the country. The Pharmacist reports to the Pharmacy Manager and works in our City, State branch.

- Evaluate and monitor drug therapy for home‑care patients in collaboration with physicians, nurses and other health‑care professionals.
- Adhere to organization policies, state and federal laws governing pharmacy practice.
- Participate in accreditation standards and quality‑assurance activities.
- Coordinate new referrals and admissions through collaboration with nurses, customer service and distribution.
- Review, receive and verify new patient orders and document on the order form.
- Assist physicians and other healthcare professionals with: planning and implementing patient care; setting therapeutic objectives; establishing monitoring parameters; and preparing progress reports.
- Conduct and document initial and ongoing assessments to identify drug‑related problems.
- Monitor outcomes by analyzing laboratory and clinical data.
- Provide consultation and answer inquiries regarding drugs and their use.
- Maintain thorough documentation of all clinical activities in the patient record.
- Process and check all medications, supplies and equipment before dispensing.
- Maintain clinical knowledge through literature review and continuing education.
- Compound sterile and non‑sterile mixtures, including parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where permitted.
- Ensure compounding/clean‑room areas and equipment meet company policies, USP 797/800 standards.
- Document all activities on appropriate logs.
- Oversee pharmacy‑technician functions as required by pharmacy‑board rules.
- Bachelor of Science in Pharmacy required;
Doctor of Pharmacy preferred. - Registered Pharmacist license in state(s) of practice.
- Minimum of two (2) years recent experience in an acute‑care setting or one (1) year recent experience in home infusion therapy.
- Experience with compounding infused drugs.
- Proven record of evaluating and monitoring drug therapies for home‑care patients.
- Good organizational and time‑management skills.
- Membership in professional societies desirable.
- Excellent verbal and written communication skills.
- Flexible schedule with ability to work on‑call evenings, weekends as required.
- Travel: 0–25 % (minimal).
Constant sitting and typing; occasional standing, walking, pushing/pulling and lifting/carrying 10–30 lbs.
