Responsibilities
  • Review and monitor patient's medication therapy and orders written for safety and efficacy considering specific patient conditions and health status; advise patient care team on patient s medication therapy and document such interventions.
  • Order verification and performing medication profile reviews including, but not limited to, checking for duplications, drug- drug and drug-food interactions, allergies, drug appropriateness, dosing, and frequency.
  • Prepare, label, and dispense medications for patients in accordance with physicians  prescriptions; ensure compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and narcotics and the maintenance of required records.
  • Check High Risk IVs, renal dose adjusting medications, IV to PO conversions, initiates/modifies/discontinues therapy, and mediates black box warning interventions.
  • Ensure that appropriate medications are available for patients and properly procured, prepared, stored, and maintained.
  • Supervise the preparation, maintenance, and dispensing process of drugs and supplies, including technical personnel performing these functions.
  • Maintain relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission standards.
  • Provide for collaborative practice management as per facility-approved protocols for anticoagulation, parenteral nutrition, vancomycin, and aminoglycoside consults, etc.
  • Assist with medication reconciliation and clarification of unsafe and unclear drug therapy orders. Provide education to patients on the use of medications as needed.
  • Participate in the education and training of healthcare professionals (e.g., physicians, nurses, pharmacy technicians, and students) in relation to medication use.
  • Demonstrate regulatory compliance as evidenced by timely completion of nursing unit inspections, as needed.
  • Maintain professional competence with the pharmacy department and state licensing requirements via staff development, independent study, and/or professional activities.
Education

Required:

Pharm.

D. or BS in Pharmacy

Experience

Preferred:
Hospital experience

Certifications

Required:

Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities. Preferred:
Board Certification in Pharmacotherapy (BCPS), CGP, etc.
