Position: Social Worker, LMSW II (Geriatric ED)

Job Summary

Provides individual/group therapy/education/support services to older adult patients and their families presented to the Emergency Department. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. Works collaboratively with the geriatric interdisciplinary team to affect positive patient and family outcomes.

Applies theoretical learning, policies and procedures. Carries out a plan of service at a basic level. Works closely with Supervisor for guidance and supervision.

Skills
  • Knowledgeable of Social Work principles and standard practices.
  • Skilled in areas of communication at various patient, family, hospital, and community levels, and the use of skills in rapport establishment, engagement of the patient and family members, focusing, negotiation, conflict mediation, facilitator and advocacy within a patient, family, and Care Team centered approach to care in an acute care setting – a bio-psychosocial perspective to patient care.
  • Effective oral and written communication skills.
  • Knowledgeable of community resources.
  • Able to work with people of all social, economic, and cultural backgrounds.
  • Flexible, open-minded and adaptable to change.
  • Ability to work under pressure.
  • Mastery of organizational and prioritizing capacities to be able to apply in a fast pace setting.
  • Demonstrated ability to connect patients and families with appropriate services utilizing a patient-centered approach to care.
  • Demonstrates cultural sensitivity with approach to care.
  • Able to communicate orally and in writing in English and Spanish to include therapeutic counseling interventions.
Work Experience

Four years of professional experience in social work is required, preferably in a hospital setting.

License/Registration/Certification

Social Work License (LMSW) in the state of Texas is required.

Education and Training

Master degree in Social Work is required.
