Position Overview

We are seeking an experienced, self‑driven Full‑Time / Part‑Time (Per Diem)
Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an OT, you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll work closely with our Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Responsibilities
  • Provide, coordinate, and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status in the patient's home.
  • Regularly evaluate and re‑evaluate patient status.
  • Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
  • Initiate, develop, implement, and revise the patient's plan of care (POC).
  • Address all problems identified in the POC.
  • Provide educational information to patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
  • Communicate with other disciplines involved in the case as warranted.
  • Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
  • Comply with all company policies and procedures to ensure high standards of care.
Requirements
  • Current OT License to practice in California - Required
  • Experience in Home Health or Hospice care - 1 Year Required
  • Current CA Driver's License
  • Able to travel within a 15 - 25 mile radius on a regular basis.
