Position: LVN - FT - H

Pay Range: $38.00 - $49.00 an hour (based on experience)

Responsibilities
  • Administrative Functions:
    Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines; ensure compliance with facility policies and procedures; review and revise department policies, procedure manuals, and job descriptions; plan shifts, services, programs, and activities; ensure Nursing Service Procedures Manual is current and followed; provide reports and recommendations; coordinate nursing services with resident services; participate in quality assurance and facility surveys;

    assist in discharge planning; interpret policies to staff, residents, visitors, and agencies; admit, transfer, and discharge residents; complete incident reports; maintain census and related administrative tasks; perform administrative duties such as forms, reports, and charting; protect resident information and system security.
  • Charting and Documentation:
    Complete and file required records upon admission, transfer, or discharge; encourage physician review and signature of orders; transcribe orders; document nursing notes; handle accident/incident reports; communicate diet orders to Food Services; resolve discrepancies in physician orders and charting; maintain transfer and discharge documentation; ensure proper documentation for unauthorized discharges; perform routine charting and sign entries.
  • Drug Administration Functions:
    Prepare and administer medications as ordered; verify resident identity; maintain narcotic records; report discrepancies; manage medication cards and adherence to stop orders; dispose of drugs per policy.
  • Personnel Functions:
    Participate in evaluations; determine staffing needs; supervise and lead nursing personnel; ensure trained staff; manage assignments; conduct rounds; ensure compliance with policies; foster interdisciplinary collaboration; address grievances; manage on-shift communications and safety.
  • Nursing Care Functions:
    Inform staff of admissions and room assignments; greet and orient residents; coordinate diagnostics and therapies; review care plans; ensure care plan adherence; supervise CNA actions; coordinate with social and activity programs; notify physicians of changes; implement restorative and rehabilitative programs; maintain clean and safe care areas; perform professional nursing tasks; use restraints per policy; collect specimens; monitor vital signs; support discharge planning and family communication;

    arrange transportation; handle post-mortem procedures as needed.
  • Safety and Sanitation:
    Enforce safety regulations and dress code; training on exposure to blood/body fluids; ensure PPE availability and sanitary work areas; participate in infection control and isolation procedures; report hazards; ensure safe operation of equipment; participate in drills and safety programs.
  • Equipment and Supply Functions:
    Advise on unit needs; maintain stock levels; establish safe operation procedures; ensure trained operation of equipment; minimize waste; maintain MSDS records for hazards.
  • Care Plan and Assessment Functions:
    Review and adjust care plans; ensure nurses’ notes reflect care; assist in MDS planning and RAPs; coordinate with care planning coordinators.
  • Budget and Planning Functions:
    Report suspected fraud related to false billings and related items.
  • Miscellaneous:
    Provide data to QA&A Committee as requested.
  • Supervisory Requirements: LPN/LVN carries administrative authority and accountability for assigned duties.
Qualifications
  • Education and Experience:

    Nursing degree from an accredited college or university, or graduate of an approved LPN/LVN program; current, unencumbered active license in this state.
  • Licenses and

    Certifications:

    Current CPR certification; knowledge/experience with PCC preferred; continuing education/licensing maintained; good standing with State Board of Nursing.
