Een zorgorganisatie in Nederland zoekt een Thuiscoach met SKJ registratie in Eindhoven. Je biedt praktische begeleiding aan cliënten en ondersteunt hen bij dagelijks functioneren. Deze rol vereist een diploma social work, rijbewijs en goede communicatieve vaardigheden. Je werkt in een team en met diverse partners om de levenskwaliteit van cliënten te waarborgen. De duur van het contract is in overleg voor 24 tot 36 uur met mogelijkheden tot vast dienstverband.
