Job Type/Status: Part Time

Position Summary:

Center For Family Services is looking for Case Manger for our Supportive Services in Affordable Housing Program. The Case Manager will be responsible for providing case management services for residents of apartment sites and coordinate workshops and training sessions for residents.

Duties and Responsibilities

• Provide and coordinate social services within the apartment complex
• Provide outreach and connect residents with the case management services provided on-site
• Coordinate referrals to community resources
• Coordinate the planning and execution of a minimum of on-site workshops/events that focus on an identified need of the resident community
• Develop with the Community Manager an identified space on-site for information sharing regarding community resources and services. Ensure the information table is kept current with brochures.
• Provide a monthly program report to the Apartment Manager.
• Conduct periodic surveys to further identify the needs of the population.
• Other duties as required.

Requirements:

• High School Diploma or GED, Associate degree preferred
• Experience in case management
