Position: Intensive Case Manager - SAIF (1395)

Location: Egg Harbor Township, NJ

Job Type/Status: Full-Time

Salary: $20.00 - $25.00 per hour

Position Summary

The position for Intensive Case Manager will be responsible for interviewing clients, obtaining pertinent information and conducting a needs assessment in order to develop an Individual Service Plan. The Case Manager will establish program goals for the client and make appropriate referrals to programs and/or other agencies to address the individual needs of the client.

Duties and Responsibilities
  • Interviews applicants, obtains pertinent information and conducts a needs assessment. Provide a comprehensive assessment and develop an ISP. Make referrals to programs and other agencies specifically geared to the needs of each individual client.
  • Establishes program goals for clients and monitors client's progress.
  • Meet regularly with clients at least twice a month to discuss and evaluate their progress.
  • Facilitate individual and group discussions to ensure client's attitudes and motivations are progressing.
  • Provide follow up and re-assessment of client's progress and service needs.
  • Completes and articulates discharge/closing of client's case when exiting the program.
  • Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same.
  • Regard all information about clients as confidential.
  • Prepares case presentations for the supervisor/manager to conduct case reviews.
  • Attends community meetings with representatives of other agencies.
  • Carry out designated duties as assigned by Supervisor/Manager.
Requirements
  • Bachelor of Social Work degree or comparable Bachelor's degree from an accredited college or university, AND a minimum of 2 years work experience.
  • Experience working with a chronically impaired population.
  • General knowledge of the resources available in the communities to be served by this program.
  • Basic interviewing, assessment, and case management skills.
  • Dependability and ability to work well with others.
  • Sensitivity in functioning in a multi-cultural work setting.
  • Ability to communicate clearly and consistently, both written and orally.
  • Sensitivity and understanding in human relations.
  • A valid NJ driver's license.
