Advanced Practice Nurse; PRN

The Advanced Practice Nurse will be responsible for, but are not limited to:

• Conduct re-certification visits to patients on hospice in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, ongoing aggressive treatment).
• Consult with the referring physician, the acute care team, and other members of the interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Master's Degree in Nursing

• Must have a Nurse Practitioner's license.
• Must have 2-4 years experience working within hospice or related field.
• Must have a Master's Degree.
• Must have a valid driver's license and a reliable source of transportation.
• Must be willing to travel.
