DescriptionThe Registered Nurse is a member of the interdisciplinary team working in collaboration with the Physician. The RN utilizes the nursing care process consistent with their license and/or certification, including initiation of culturally competent care across inpatient, outpatient and community settings. The RN coordinates the interdisciplinary plan of care and ensures alignment with the medical treatment plan.
The patient care process includes the following:
- Assessment – identify, gather, and organize subjective and objective data pertaining to the patient/family/significant other (physical, psychosocial, spiritual and cultural).
- Planning – identify actual or potential problems and desired outcomes (goals) and document a plan of care to guide the patient/family/significant other and the interdisciplinary team.
- Intervention – implement interventions or orders in accordance with the plan of care and medical treatment plan.
- Evaluation – determine the effectiveness of interventions and whether outcomes are achieved; may trigger reassessment and revisions to problems, goals, plan of care, and interventions; documentation of required actions.
Based on assessment findings, patients are prioritized relative to biophysical, psychological, environmental, self-care, educational and social status. Goals are identified to provide direction for patient care and health promotion. Education is aligned with identified problems and goals to promote patient empowerment. The employee may be required to perform Limited Laboratory Services as described in the job duties.
Licensed and registered care providers responsible for utilizing the care process within their scope of practice may document the assessment, plan, intervention and evaluation. All patient care activities are administered according to standards, guidelines and organizational policy. The RN actively supports performance improvement and provides leadership to the patient care team. Through educational activities, the RN assumes responsibility for ongoing professional growth.

- Coordinate and document the assessment, plan, intervention and evaluation within unit/department standards.
- Ensure care is consistent with the medical treatment plan and interprofessional team goals.
- Promote patient education and empowerment aligned with identified problems and goals.
Education/License: Graduate of an accredited school of nursing; BSN preferred. Current RN licensure in New Jersey or valid multi-state license. Basic Life Support (BLS) required. Valid NJ Driver's License required for all employees in the Adult and Child Partial Programs.
Experience: Proficiency in Clinical Applications preferred at time of hire; training provided with 30–60 days to achieve competency. Various specialty-specific requirements listed below may apply.
