Social Services Specialist

This is a part time position scheduled for 3 days per week with a flexible schedule, on-site role located in Egg Harbor, New Jersey, United States. The Social Services Specialist will be responsible for providing support to individuals and families by assessing their needs and connecting them with appropriate community resources. Daily responsibilities include case management, facilitating community outreach efforts, delivering social services, and collaborating with other healthcare and support professionals to develop individualized care plans.

Background

• in Social Work, Social Services, and Human Services
• Experience with Case Management and creating individualized care plans a plus but training will be provided
• Excellent interpersonal and communication skills to work compassionately with diverse populations
• Bachelor’s degree in Social Work, Human Services, or a related field;
  Master’s degree preferred
• Familiarity with healthcare settings or rehabilitation services is a plus
• Exceptional organizational and problem-solving skills
