As Administrative Agent for the Program, Vital Check Wellness is seeking an Advanced Practice Provider NP/PA to provide the following services to one or more clients of the Program. A Nurse Practitioner/Physician Assistant with dermatology experience to provide skin cancer screenings and general dermatological evaluations onsite at a private business in DuQuoin, IL. Vital Check manages administrative logistics and facilitates scheduling, allowing clinicians to focus on patient care.
This is an excellent opportunity to practice preventive and personalized medicine in a supportive, structured setting. Vital Check does not provide medical care and does not control clinical decision‑making.
Responsibilities- Provide skin checks and skin cancer screenings.
- Consult with patients to understand their dermatological health concerns.
- Advise/refer patients for follow-up care.
- Active, clean Nurse Practitioner/Physician Assistant license to practice in Illinois.
- Active BLS/ACLS certification required.
- Minimum 3 years of direct dermatology experience following postgraduate training preferred.
- A collaborating physician is necessary unless state requirements for independent/autonomous practice are met.
- Proficient with computers, customer service‑oriented, and able to work independently.
- Patient education driven.
- Personal laptop or tablet required.
- Must be able to reliably commute to Du Quoin, Illinois 62832.

licensed physicians and other clinical providers deliver the clinical care and are exclusively in control of and responsible for all clinical aspects of the healthcare services. As Administrative Agent for the Program, Vital Check provides both Program clients and healthcare providers access to its proprietary technology, know-how, processes and techniques, and other administrative services including managing administrative logistics, facilitating scheduling, and collecting and processing payments.
