Your Opportunity:
Nurse Case Manager in the Chronic Kidney Disease Clinic at the Kaye Edmonton Clinic: A member of the multidisciplinary team the RN (nurse case manager) provides care to patients with stages 4 and 5 Chronic Kidney Disease (CKD). The nurse case manager is responsible for the assessment, planning, implementation and evaluation of patient care. The RN uses nursing knowledge in addition to treatment guidelines to develop a patient care plan using a case management model.
The RN reviews lab results, symptoms and other diagnostics to assess the patient’s response to care and communicates the results with the nephrologist. The RN provides education to patients including developing a learning assessment based on past health history, current health status, learning needs, self-care ability and values and beliefs around health care issues and treatment. The RN provides formal education for patients and families about Renal Replacement Therapy including Conservative Kidney Management.
The RN also participates in program development and quality improvement initiatives.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
PLEASE NOTE:
Provisional Permit holders will be paid $ - $ per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii) as applicable.
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the last five (5) years, you must complete the following free courses: CRNA Medication Management (Chapters 1-4), available at and the NextGenU Health Assessment Resources Certification, available at

Current experience in the CKD clinic (within 12 months). Proven ability to manage complex issues, multiple patients and competing demands. Previous experience in Chronic Disease management. Experience with Connect Care.
