Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Registered Nurse to join our Home Living team to provide professional support services in the homes of clients of all ages and stages of life. As a self-directed team member with Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the health care team and has responsibilities, including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes.
In addition, this position provides case management as well as direct client care that meets established clinical standards, and is evidence based and client centered. In collaboration with family and other partners, the Registered Nurse acts as a consultant on the team providing clinical expertise and direction to other team members regarding client cases, developing a care plan, based on those unmet needs, using the appropriate internal and external resources.
As a member of the multidisciplinary team, the Registered Nurse is responsible for participating in staff development and mentoring student clinical experiences. This position is based out of the Westview Home Care office but will provide support to the Suburban/ Rural West team, which includes travel to other home care network offices, as required.
- Provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.
- Utilize nursing processes with critical thinking, problem solving and decision making.
- Teach, counsel and advocate on behalf of patients and their families.
- Play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
- Provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
- Collaborate with the team as a consultant on client cases, develop care plans based on unmet needs, and coordinate internal and external resources.
- Participate in staff development and mentoring student clinical experiences.
- Travel to support to the Suburban/Rural West team and other home care network offices as required.
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
- If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses prior to employment: CRNA Medication Management (Chapters 1-4) and NextGenU Health Assessment Resources Certification.
- Two years of recent nursing experience (obtained within the last 5 years) or completion of a refresher program within 2 years.
- Self-directed and the ability to work independently and use sound professional judgement.
- Excellent organizational skills to prioritize and efficiently organize many details in a timely manner.
- Communication and assessment skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment.
- At the time of interview, the ability to demonstrate computer knowledge and proficiency via a hands-on assessment; sufficient typing speed to meet workload demand; ability to navigate Microsoft Office programs (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet.
- Physically capable of performing nursing tasks which could include transfer of clients; able to work in clients’ homes in various settings and weather conditions; able to lift and carry medical bags weighing approximately 25-30 lbs.
- Baccalaureate degree in nursing preferred; relevant experience in community nursing will be considered.
- Palliative care experience.
- Previous Continuing Care and/or geriatric experience.
- Experience with Connect Care and the RAI Home Care assessment tool.
- 2 years current Case Management Experience in Continuing Care.
