Care Manager – MSW / LCSW

$5,000 Hiring Bonus for eligible external hires that meet the required qualifications and conditions for payment.

The Clinical Social Worker partners with patients, families, and the health care team to address and advocate for patients’ social and emotional needs. The role includes psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral.
Guided by the NASW Code of Ethics and the organization’s mission.

• Conduct psychosocial assessments and develop treatment plans.
• Provide therapeutic interventions and crisis intervention.
• Coordinate discharge planning and resource referrals.
• Partner with patients, families, and interdisciplinary teams to improve care.
• Lead initiatives to achieve patient care and resource goals.

• Master’s Degree in Social Work from an accredited school.
• Washington Clinical Independent Social Worker Associate License upon hire.
• Minimum 1 year of experience in acute medical or mental health settings (inpatient or outpatient).

• ACM/CCM certification.

• Master’s Degree in Social Work.
• Washington Licensed Clinical Social Worker (LCSW) license.
• Experience in acute medical or mental health settings.
