Hackensack Meridian Health - - Responsibilities:
Provide direct resident care to ensure resident's plan of care is followed;
Complete all required documentation as per state/federal and facility guidelines;
Provide patient care per Patient Care Plan including ROM exercises, ambulation, transfer training, feeding assistance, and vital signs;
Assist with room changes and post-mortem care as needed;
Participate in committees and communicate with RN about patient status
