OverviewThe Registered Nurse functions as a caregiver for a specific patient population, providing direct care as part of an interdisciplinary team and coordinating patient care with nursing peers, physicians, and support services.
Responsibilities- Deliver nursing practices according to current evidence-based research, policies, and standards of care to ensure safe, high-quality, individualized patient care.
- Assess patients, develop plans of care, implement interventions, and evaluate outcomes.
- Collect comprehensive data, perform age- and disease-specific assessments, identify changes in patient status, and tailor education to the patient/family based on age, disease, and cultural learning differences.
- Coordinate and communicate plans of care with patients, families, and the health care team; ensure timely documentation and appropriate escalation when needed.
- Provide or coordinate patient and family education; adhere to established protocols and demonstrate evidence-based nursing practice.
- Support patient discharge planning to ensure safe transition and continuum of care.
- Adhere to organizational competencies, standards of behavior, and safety requirements, including medication administration with appropriate documentation.
- Minimum education:
Associate degree in Nursing or Nursing Diploma; BSN in progress or planned within three years of hire as applicable by policy. - Six or more months of relevant nursing experience or active participation in Hackensack Meridian Health Nurse Residency Program.
- Current and valid Registered Nurse license to work at the assigned location; BLS/CPR within 60 days of hire.
- Ability to learn quickly, work within a team, and deliver high-quality patient-centered care.
- National certification in area of specialty preferred when eligible.
