The Physician Coder III is responsible for accurately abstracting data following ICD-10-CM, CPT, and HCPCS guidelines and CMS directives across the HMH network. Performs data entry of required abstracted patient information into the electronic medical record system and queries physicians when appropriate.

• Assign codes for reimbursements, research and regulatory compliance using coding guidelines and conventions.
• Code and abstract patient encounters, including diagnoses, procedures, significant reportable elements, and complications.
• Analyze medical records and identify documentation deficiencies.
• Review and verify that documentation supports existing diagnoses, procedures, and other charges.
• Identify reportable elements, complications, and other quality measures.
• Communicate with physicians to clarify information via the physician query process.
• Assign CPT, HCPCS and ICD-10-CM codes.
• Proficient in professional (profee) coding and E/M guidelines (1995/1997, 2021 update).
• Maintain knowledge of NCCI edits and NCD/LCD edits and address related issues.
• Maintain required productivity and quality standards.
• Adhere to HMH organizational competencies and standards of behavior.

• High School diploma or GED.
• Minimum of 3+ years of coding experience in trauma Level 1 and academic teaching facility settings.
• Focused background in Physician and pro-fee coding with knowledge of E/M guidelines.
• Proficient in coding office/outpatient procedures in office and outpatient hospital settings.
• Strong understanding of physiology, medical terms, and anatomy.
• Proficiency in computer skills including typing speed and accuracy.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and Google Suite.

• Multiple years of coding experience in trauma Level 1 and academic teaching facilities.
• Background in multi-specialty physician services.

• RHIT, RHIA, CCS, or CPC credential (AHIMA or AAPC).
• An approved AHIMA or AAPC coding credential.

• An approved AHIMA or AAPC coding credential.

Starting at $34.65 hourly. HM H is committed to pay equity and transparency. The posted rate is a good faith estimate of the minimum base pay for this role at the time of posting and does not reflect the full value of our total rewards package.
