Job Summary
POSITION SUMMARY: Assessment Coordinator shall provide administrative support to the patient admission process. The incumbent contributes to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion.
Qualifications- Graduate of an accredited school of nursing.
- Current active registered nurse licensure in the State of Texas.
- Current BLS Certification.
- Valid Texas Driver’s License.
- Willingness to be cross‑trained to other areas.
- Strong organizational skills.
- Good written and verbal communication skills.
- Ability to read, write, and speak English.
- Clear communication with nursing, administration, and physicians.
- Prefer hospital experience.
- Three to five years in a health‑care setting strongly preferred.
- Strong communication and organizational skills.
- Reasoning ability and good independent judgment.
- Professional image.
- Ability to work with frequent interruptions.
- Promote the facility’s mission, vision, and values.
- Arrange pre‑admission screening within 30 minutes of referral.
- Orient patients and families to units.
- Confirm eligibility within one business day of referral.
- Complete verification of admission pre‑certification and insurance benefits.
- Coordinate bed availability with Nurse Manager / Program Director.
- Develop network with case managers for referrals and discharge planning.
- Communicate with hospital case managers daily regarding post‑acute needs.
- Conduct center tours for patients and families.
- Attend team business meetings and contribute ideas/solutions.
- Provide in‑service/cross‑training on admissions knowledge and skill areas.
- Contribute to system improvements.
- Assist other staff and demonstrate flexibility for shared team functions.
- Complete tasks as requested.
- Utilize resources appropriately and maintain equipment.
- Attend scheduled unit quality management meetings.
- Submit data as requested.
- Assist with coverage for Case Management/Social Work as needed.
- Adhere to safety standards and encourage compliance.
- Perform other duties as assigned.

Physical Requirements
:
Light/moderate lifting up to 30 lbs from floor to shoulder height; lifting more than 30 lbs with assistance; walking, standing, coming to a halt; kneeling, squatting, stooping/bending, and reaching above shoulder height; climbing stairs; operating office and mechanical equipment; ability to read, write, count, see, hear, and comprehend verbal and written communication. Work largely in an air‑conditioned environment with minimal exposure to body fluids.
