POSITION SUMMARY:
Responsible for providing nursing services to terminally ill patients in their home setting and supportive services to family members in compliance with all applicable federal, state and local health and safety codes including LSC®, laws and regulations, and all applicable policies and procedures of hospice
POSITION EDUCATION/ QUALIFICATIONS:
- Graduate of accredited school of nursing required. BSN preferred.
- Current registered nurse's license in the State of Texas
- Documentation of at least one (1) year or equivalent of nursing experience in an acute care setting.
- Current CPR Certification required.
- Valid and current Driver's License required
- Valid and current proof of Auto Insurance required.
- Hospice or home health experience preferred.
- Knowledge of local, state, and federal laws for delivery of hospice services preferred.
- Demonstrate ability to relate effectively with physicians, health care professionals, the community, patients/families, and members of the hospice interdisciplinary team.
- Three to five (3-5) years' experience is required,
- Must demonstrate an understanding of community health, welfare, and social agencies.
- Working knowledge of Medicare, Medicaid and third party payers preferred.
- Hospital or acute care setting experience is strongly preferred.
POSITION RESPONSIBILITIES:
- Supplement the nursing care needs of the patient as provided by the RN and as specified by the patient's plan of care.
- Assist the RN in carrying out the patient's plan of care.
- Participate in gathering assessment data.
- Ensure services are provided in accordance with organizational policies and procedures and in accordance with the LVN scope of practice.
- Assist in the formulation of goals/outcomes and a plan of care in collaboration with the RN, patient, family and IDT members.
- Assist the RN and physician in performing specialized duties related to end of life care.
- Prepare clinical and progress notes that demonstrate progress toward established goals according to policy and procedure guidelines and as defined by the plan of care.
- Prepare equipment and materials for treatment adhering to aseptic technique as required.
- Assess patient/family response to care.
- Assist the patient in learning appropriate self-care techniques.
- Maintain awareness of verbal and non-verbal patterns of communication of patients/families.
- Support the management of discomfort and symptom relief utilizing specialized nursing skills related to palliative and end of life care.
- Educate the patient and family regarding the disease process, self-care techniques, and issues related to end of life care.
- Demonstrate knowledge of medication administration, interactions and side-effects.
- Demonstrate knowledge and competence in pain and symptom control concepts, interventions.
- Communicate information using current processes and technology available.
- Ensure Medical Director, attending physician, and other personnel providing care are informed of changes in the patients' condition as appropriate.
- Evaluate own needs for support and identify systems to meet the needs.
- Act as physician agent when calling in prescriptions for patients by following agency protocols.
- Participate in the QA/PI process.
- Participate in clinical call as scheduled.
- Monitor staff and patient safety.
- Participate in IDT/IDG staff meeting.
- Participate in orientation of new staff and volunteers as requested.
- Participate in in-service programs.
- Maintain current level of skills and evidence of competency.
