Position Title: Senior LTSS Service Care Manager (RN).
Work Location: Hybrid (2 days remote, 3 days member-facing, sourced from McAllen, Edinburg).
Assignment Duration: 6 months with possible extension.
Job DescriptionFace-to-face assessments are completed three days a week, with two days devoted to documentation. Tasks include completing forms and assessments within turnaround times and conducting outreach activities.
Candidate Requirements- Education / Certification: Required: RN;
Preferred: n/a. - Licensure: Required: RN;
Preferred: n/a. - Experience: PDN experience, Pediatric experience, Home Health.
- Must haves: Pediatric experience.
- Nice to haves: Home Health.
- Disqualifiers: No experience in Home Health.
- Performance indicators: n/a.
- Best vs. Average: Case management and Home Health.
