Triage Nurse-Primary Care – Almara Women’s Health

The Triage Nurse provides healthcare information, education, recommendations, and general nursing care to patients in the clinic setting. Interacts with patients, physicians, physician assistants, nurse practitioners, clinic staff, and non-Almara clinic personnel by phone, email, fax, and in person. This is a part-time role, FTE .85, Monday – Thursday: 8:00 AM – 5:00 PM out of our Edina location. Almara Women’s Health is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.

• Triage incoming patient calls, route calls to providers per established protocols, and communicate provider recommendations to patients.
• Review laboratory and diagnostic test results and relay information to patients in accordance with clinical protocols and provider guidance.
• Partner with providers on population health initiatives, including monitoring and supporting improvement of HEDIS measures.
• Track diagnostic tests, referrals, and consultations with external (non-Almara) healthcare providers to ensure timely follow-through per provider orders.
• Manage prescription refill requests in accordance with clinical protocols, including completing medication prior authorizations.
• Conduct RN phone visits as appropriate, including hospital discharge follow-up, patient education, and case management.
• Provide health promotion, patient education, and coaching related to wellness, disease prevention, and chronic disease management.
• Collaborate in developing, implementing, evaluating, and revising clinical policies, procedures, protocols, and guidelines. Update existing triage protocols and draft new ones in collaboration with providers as needed.
• Assist in coordinating home healthcare orders and related communication.
• Serve as a resource to providers and clinic staff by offering clinical information and support as requested.
• Document all patient interactions and communications accurately, consistently, concisely, and in a timely manner.
• Provide assistance with Medical Assistant responsibilities when additional support is needed.
• Submit sexually transmitted disease (STD) reports to the Minnesota Department of Health as required.
• Work independently with minimal supervision.

• RN:
  Diploma graduate from an accredited school of nursing;
  Bachelor’s Degree (BSN) or Associate’s Degree (ADN) in nursing from an accredited college; current or pending reciprocity, Minnesota State Board of Nursing license.

The pay range displayed reflects full-time compensation and will be prorated for employees working less than a full-time schedule.
