The Triage Nurse provides healthcare information, education, recommendations, and general nursing care to patients in the clinic setting. Interacts with patients, physicians, physician assistants, nurse practitioners, clinic staff, and non-Almara clinic personnel by phone, email, fax, and in person. This is a part-time role, FTE .85, Monday – Thursday: 8:00 AM – 5:00 PM out of our Edina location. Almara Women’s Health is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
EssentialJob Functions
- Triage incoming patient calls, route calls to providers per established protocols, and communicate provider recommendations to patients.
- Review laboratory and diagnostic test results and relay information to patients in accordance with clinical protocols and provider guidance.
- Partner with providers on population health initiatives, including monitoring and supporting improvement of HEDIS measures.
- Track diagnostic tests, referrals, and consultations with external (non-Almara) healthcare providers to ensure timely follow-through per provider orders.
- Manage prescription refill requests in accordance with clinical protocols, including completing medication prior authorizations.
- Conduct RN phone visits as appropriate, including hospital discharge follow-up, patient education, and case management.
- Provide health promotion, patient education, and coaching related to wellness, disease prevention, and chronic disease management.
- Collaborate in developing, implementing, evaluating, and revising clinical policies, procedures, protocols, and guidelines. Update existing triage protocols and draft new ones in collaboration with providers as needed.
- Assist in coordinating home healthcare orders and related communication.
- Serve as a resource to providers and clinic staff by offering clinical information and support as requested.
- Document all patient interactions and communications accurately, consistently, concisely, and in a timely manner.
- Provide assistance with Medical Assistant responsibilities when additional support is needed.
- Submit sexually transmitted disease (STD) reports to the Minnesota Department of Health as required.
- Work independently with minimal supervision.

- RN:
Diploma graduate from an accredited school of nursing;
Bachelor’s Degree (BSN) or Associate’s Degree (ADN) in nursing from an accredited college; current or pending reciprocity, Minnesota State Board of Nursing license. - Previous Primary Care experience

The pay range displayed reflects full-time compensation and will be prorated for employees working less than a full-time schedule.
Working Conditions- Frequently lift up to 50 pounds independently and occasionally up to 100 pounds with assistance.
