Job Overview
Ebenezer is looking for an Admissions Coordinator II to join our corporate admissions team at the Ebenezer Corporate Office in Edina, MN. This Nursing (LPN/RN) position will develop relationships with referral sources and work with internal teams to promote and maintain census at our Skilled Nursing Facilities in Minnesota. Assist the Social Service Department in meeting the needs of the residents and families of the care center.
This admissions schedule includes:
- On Call/Casual hours
- Day shifts available
- To maintain Casual status, employees must be willing to work a minimum of three (3) shifts per month (two shifts must be weekend shifts) and one (1) Holiday shift per year. The weekend starts with the beginning of the night shift on Friday and ends with the beginning of the night shift on Sunday.
- Process and complete patient admissions.
- Provide pre-admission counseling, information, and tours.
- Clinically evaluate referrals to ensure all needs can be met from a medical perspective.
- Facilitate the intake process and secure primary care provider.
- Review EPIC for proper documentation and orders.
- Contact pharmacy as needed to verify cost of medications.
- Determine need for equipment and supplies and coordinate with internal team.
- Facilitate PAS Screening and Level II as needed.
- Coordinate admission process with resident, resident representative, community providers, and staff as appropriate.
- Set up interpreter services as needed.
- Ensure completion of admission paperwork.
- Inform residents and resident’s representatives of financial responsibilities, payment options, and agency contacts according to state and federal regulations.
- Build and maintain relationships with referral sources and assist with marketing and outreach activities as appropriate.
- Maintain internal and external waiting lists as appropriate.
- Verify necessary payer source(s) as appropriate.
- Coordinate room change process as appropriate.
- Associate of Arts in Vocational/Technical Training or an Associate Degree.
- One to three years of previous experience.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Bachelor of Science in Medical or Human Service Field.
- Three to five years of previous experience.
- Two years of experience in LTC.
- Computer skills.
- Experience in Medical or Human Service Field.
