Are you ready to be at the forefront of transforming community-based neurological rehabilitation?
We're seeking a forward-thinking and experienced Allied Health Professional Physiotherapist or Occupational Therapist, to lead the Community Neurological Conditions management team (CNCMT) at the Royal Free Neurological Rehabilitation Centre (NRC). This team plays a vital role in supporting rehabilitation and care pathways for adults with long-term neurological conditions across the borough of Barnet.
The CNCMT delivers specialist domiciliary and outpatient care for people with neurological conditions (excluding Stroke and Parkinson's disease). Based within the NRC at Edgware Hospital, the team has strong links with acute and community services. Multidisciplinary collaboration, evidence-based practice, and staff wellbeing are central to our ethos.
You will:
Lead a multidisciplinary team of experienced clinicians
Be responsible for day-to-day operations and ongoing service development and team leadership
Provide expert clinical input in neurological rehabilitation.
Represent the CNCMT as part of the NRC senior leadership team

Main duties of the job- Lead the Community Neurological Conditions Management Team (CNCMT), comprising 25 multidisciplinary staff.
- Act as a member of the clinical leadership team within the NRC.
- Oversee day-to-day service coordination, review and planning in collaboration with the MDT and clinical lead.
- Manage CNCMT staff operations, including recruitment, supervision, and appraisals.
- Ensure high-quality therapy service delivery within the community team.
- Provide expert knowledge for staff supporting patients with complex and progressive long term neurological conditions.
- Lead the continued development of CNCMT, promoting integrated working with internal and external partners.
- Support the NRC clinical lead with service development, clinical governance, and workforce initiatives at the NRC.
- Work with NRC Clinical lead for NRC to lead on improvements for Digital transformation to improve processes, pathways and data collection.
- Maintain up-to-date clinical practice and support development of therapy staff in line with professional standards.
- Design and manage referral pathways and clinical documentation in collaboration with the MDT.
- Lead on short- and medium-term workforce planning within the community team in conjunction with NRC Clinical lead.
- Contribute to the development of integrated care pathways between secondary and primary care, working closely with local providers, NCL ICB, and the wider service network.

Education and Qualifications- Recognised Degree Qualification or equivalent (Post-Graduate Diploma or MSc in Occupational Therapy or Physiotherapy leading to registration with HPC)
- Health Professions Council - License to Practice
- Registered Member of Royal College of Occupational Therapists or Chartered Society of Physiotherapists.
- Evidence of successful completion of specialist courses at / equivalent to MSc level in
