JAG Physical Therapy is seeking to hire a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic-related conditions. Our mission is to help patients regain mobility, strength, function and confidence in the comfort of their own homes. This role requires evaluating and treating patients in their home as part of the plan of care, with the goal of transitioning to a JAG outpatient orthopedic location for continuity of care.

• Bachelor’s or Master’s Degree required;
  Doctorate degree preferred
• Licensed Physical Therapist in the state where practicing
• Completed state-regulated CEU requirements (NY, NJ)
• Valid CPR certification
• Experience/knowledge with EMR software is preferred
• Excellent oral and written communication
• Strong clinical decision-making skills
• Excellent work ethic and dedication to patient success
• Basic computer skills
• Team player with ability to work independently
• Positive, professional demeanor at all times

• Educate patient on safe bed mobility, transfers and ambulation with post-operative precautions
• Assist with transfer, ambulation, and stair negotiation to determine discharge readiness to home
• Collect and record patient care data per guidelines of patient care services and regulatory requirements, including initial evaluation, daily notes, progress reports, discharge plans, and team communications
• Complete daily billing and follow department productivity guidelines
• Prepare for evaluations per department policies, including patient history and knowledge of diagnoses
• Select and administer tests and measures
• Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care
• Establish treatment plans, collaborate with the interdisciplinary team and patient’s support system, and reassess as needed
• Provide education/training to patient and/or support system
• Functional training including ADLs/IADLs, therapeutic activities, equipment prescription, and community reintegration
• Develop treatment plans and provide interventions that are safe, effective, and compliant with JAG policies
• Provide data and documentation using Prompt EMR for all treatments
• Attend annual JAG Billing and Coding seminars
• Coordinate with Manager/Patient Access for scheduling evaluations once demographics are received
