Overview

Fairview Health Services is seeking a Same Day Nurse Practitioner Partner to join our primary care practice at M Health Fairview Clinic – Eden Prairie
.

Practice Details:

  • 8 primary care providers
  • Patient encounters average 18-22 patients per day
  • 34 patient contact hours per week
  • Openings: 1.0 FTE Flexible schedule
  • As an Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
  • Fully integrated Electronic Medical Record (EPIC).
Responsibilities
  • Assess health status for specific patient populations using advanced practice knowledge and skills.
  • Obtain relevant health and medical history.
  • Perform a physical examination based on age and medical history. Perform/order preventative and diagnostic procedures based on the patient's age and history.
  • Identify health and medical risk factors.
  • Identify acute and chronic health problems.
  • Order and accurately interpret diagnostic results.
  • Develop and implement an individualized plan of care.
  • Establish practice agreements with collaborating physician.
  • Collaborate with health care team members in developing realistic care expectations.
  • Determine effectiveness of treatment plan with documentation of patient care outcomes.
  • Revise and modify the care plan as needed to achieve health and medical goals.
  • Refer care to other health care professionals and community agencies as needed.
  • Prescribe pharmacologic (including, when appropriate, controlled substances) and non-pharmacologic agents.
  • Communicate effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care.
  • Provide education to patients, families or caregivers as needed.
  • Document according to established guidelines to facilitate ongoing quality care in a timely manner.
  • Respond in a timely manner to phone calls regarding patient needs.
  • Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
  • Participate in access planning for applicable area or population.
  • Support appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures.
  • Adhere to all relevant policies, procedures, and practice guidelines.
  • Assist with the preparation for JCAHO and other regulatory surveys.
  • Manage patient care through appropriate use of available resources.
  • Appropriately prescribe and order ancillary services.
  • Utilize Fairview referral network appropriately.
  • Manage chronic health/disease conditions.
  • Provide leadership for clinical and support staff.
  • Serve as a resource to support staff, enhancing quality of care.
  • Participate in the training, orientation, and evaluation of clinical and support staff.
  • Participate in appropriate clinical investigation relevant to specific patient population.
  • Respond to nursing staff educational needs in formal and informal venues demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities.
  • Serve as consultant and role model for nursing staff.
  • Maintain competence and professional development. Engage in continuing education.
  • Be active in professional and other appropriate organizations.
  • Demonstrate behaviors that support performance improvement activities.
  • Demonstrate ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of populations served.
Required Qualifications
  • Graduate of an accredited school of nursing and Nurse Practitioner training program.
  • Board certified or board eligible through AANP or ANCC.
  • Licensed or eligible for licensure through the Minnesota Board of Nursing.
  • Ability to obtain and maintain DEA certification in the state of Minnesota.
  • Must meet credentialing and privileging requirements.
  • Minimum two years of Primary Care Nurse Practitioner experience
