The Carers Hub Officer plays a pivotal role in supporting unpaid carers by acting as a firstpoint of contact, providing information, guidance, and access to services atlocal ABHB hospitals. The role involves managing day-to-day operations of thecarers hub, ensuring that carers receive timely, personalised support, and facilitating access to a range of resources aimed at improving their wellbeingand sustaining their caregiving relationships.
Mainduties of the job
- Act as the primary point of contact for carers via ward visits, and in-person enquiries at the Carers desk.
- Provide accurate information and referrals to appropriate services, including, health, financial, and emotional support.
- Assess carers' needs through structured intake interviews.
- Develop and maintain individual care plans in collaboration with carers.
- Monitor progress and follow up regularly to ensure carers needs are met.
- Maintain up-to-date leaflet supplies of local support services, community resources, and partner organisations.
- Organise and promote carers seasonal events.
- Completion and submission of daily evaluation log
- Keep detailed records of interactions and service outcomes.
- Prepare reports and data for internal review and external funding bodies.
- Assist in the development and dissemination of informational materials.

Qualifications:
- Previous experience in a support, case management, or community outreach role.
- Excellent communication and interpersonal skills.
- Strong organisational skills and attention to detail.
- Empathy and understanding of the challenges faced by unpaid carers.
- Ability to work both independently and as part of a team.
- Proficiency in standard office software and databases.
- Relevant qualifications in social care, community development, or a related field are desirable.

The role involves managing day-to-day operations of the carers hub, ensuring that carers receive timely, personalised support, and facilitating access to a range of resources aimed at improving their wellbeing and sustaining their caregiving relationships.
Key Responsibilities:
- Act as the primary point of contact for carers via ward visits, and in-person enquiries at the Carers desk.
- Provide accurate information and referrals to appropriate services, including, health, financial, and emotional support.
- Case Management:
- Assess carers' needs through structured intake interviews.
- Develop and maintain individual care plans in collaboration with carers.
- Monitor progress and follow up regularly to ensure carers needs are met.
- Resource Coordination:
- Maintain up-to-date leaflet supplies of local support services, community resources, and partner organisations.
- Organise and promote carers seasonal events.
- Overall responsibility of the upkeep and presentation of the Carers Hub desk.
- Administrative Duties:
- Completion and submission of daily evaluation log
- Keep detailed records of interactions and service outcomes.
- Prepare reports and data for internal review and external funding bodies.
- Assist in the development and dissemination of informational materials.
- Community Engagement:
- Build and maintain relationships with key hospital wards, health services, and hospital teams.
- Represent the carers hub at meetings to raise awareness of services available to carers.
- Solicit and incorporate feedback from carers to enhance service delivery.
- Contribute ideas for improving operations and outreach strategies.
