The Community Coordinated Entry Lead (CCEL) serves as the main point of contact for homeless individuals, families, and service providers within the Dairyland Housing Coalition. The CCEL is responsible for conducting in-depth assessments of needs and barriers of homeless individuals and families, connecting them to a housing program operated by Western Dairyland or agency partners.
This position is responsible for collecting all documentation needed for placement into a housing program and ensuring clients waiting for housing services remain engaged and available for when a housing program has an opening, as well as pulling the housing prioritization lists and filling programs. The CCEL may also help confirm eligibility for programs, manage the Coordinated Entry and Expanded Coordinated Entry Prioritization Lists, and oversee follow-up including ensuring the referral to the housing program is completed successfully.
This is a full-time, 36 hours per week, four-day workweek position. The wage range is $21.51 to $28.68 per hour.
Essential Functions:
- Assess the needs of people experiencing homelessness or at risk of homelessness.
- Provide housing navigation assistance to individuals identified in the coordinated entry process who do not have an existing case manager and would like assistance from a housing program.
- Administer the pre-screen, assessment tools, and all necessary paperwork.
- Assist clients with gathering basic documentation required for housing.
- Provide case management services to build ongoing rapport.
- Assist clients in identifying potential housing barriers and service needs and connecting them to appropriate resources to address those barriers and needs wherever possible.
- Serve as the primary point of contact for individuals waiting for housing services.
- Become familiar with the housing and support programs currently participating in the Coordinated Entry system.
- Collect and maintain client level information as needed to best assess needs and vulnerability while maintaining dignity and respect for participants privacy, time, and other considerations.
- Frequently interact with people with diverse racial, cultural, and economic backgrounds, gender identity, and sexual identity and treat all community members with dignity and respect.
- Enter all data into HMIS and non-HMIS in a timely manner and abide by all policies and procedures.
- Provide outreach services to go beyond those who present for assistance.
- Provide support to local agencies who participate in HMIS.
Coordinated Entry Lead Tasks
- Serve as the local Coordinated Entry expert.
- Ensure a consistent and accurate flow of information between the WI BOSCOC Board of Directors, or other designated entity, and the LCES and local homeless coalition(s).
- Run reports in HMIS and non-HMIS.
- Ensure all participating agency staff within the LCES have completed required trainings.
- Attend all Coordinated Entry trainings.
- Maintain all copies of all Agency and Staff Agreements.
- Ensure marketing and outreach activities are occurring as required by WI BOSCOC policy.
- Ensure LCES has established an after-hour plan for accessing emergency services.
- Ensure all relevant stakeholders are aware of the plan.
- Ensure the plan is updated annually.
Coordinated Entry List Holder Tasks
- Maintain the HMIS and Non-HMIS List.
- Upon request from a housing project, provide the highest prioritized person’s unique identifiers, prioritization information (assessment scores, length of time homeless, chronic homeless status), and the referring agency contact information.
- Respond to requests from participating agencies and staff in a timely manner.
Operations Management Tasks
- Serve as the central point of contact for homeless intakes.
- Represent Western Dairyland at appropriate community meetings and forums.
- Prepare written reports and other necessary documentation, as needed.
- Provide data regarding homelessness as requested.
- Provide trainings to all No Wrong Door agencies.
- Monitor coordinated entry of local agencies and provide support and documentation for agency monitoring visits
