Under general supervision of the Administrator, the RN assesses the need of clients for personal care services according to established nursing process of diagnosing, planning, intervention, and evaluation. Supervises the care of clients living at home, trains and supervises Personal Care Workers (PCWs), works with a diverse population which includes individuals whose primary language is not English, collaborates with nursing team and clients’ primary care providers to establish a plan of care, consult with Director of Billing regarding clients’ PA, make referrals and follow-up.
RN will be expected to travel outside of home community to provide training as needed.
This description will be reviewed yearly and may change from time to time as required by the need of the agency. Other duties may be assigned.
- Evaluate the need for services and make referrals to other services as appropriate.
- Secure order from the client’s physician.
- Develop a Plan of Care giving full preference to client’s preferences for service arrangements and choice of PCW.
- Interpret the Plan of Care to PCW.
- Schedule weekly home visits.
- Adhere to home visit schedule that is prepared on a weekly basis.
- Develop appropriate time and service reporting mechanisms for PCW and teach how to use it.
- Provide written instructions to PCW about the services to be performed.
- Demonstrate to PCW how to perform the personal care services.
- Train or supervise the training of PCWs.
- Provide additional training to PCW when indicated by PCW performance or competency.
- Provide PCW supervision at least every 60 days.
- Perform PCW performance evaluation annually and report to Human Resource.
- Enter data in the online screening tool.
- Complete Plan of Treatment and Prior Authorization forms.
- Update client’s status every 60 days or sooner if there are changes.
- Promptly notify client’s physicians or other appropriate medical personnel and legal representative of any significant changes in client’s condition, observed or reported.
- Review physician order every 60 days or as client’s case requires, but no less than once a year.
- Review and sign PCW time sheet every payroll.
- Perform monthly audits of clients’ medical records.
- Communicate with Primary Care Providers to update information and ensure quality care.
- Educate clients and their families to ensure informed decision-making regarding their care.
- Attend all required staff meetings and in-service trainings.
- Participate in monthly clients’ service update meetings.
- Comply with all OSHA and HIPAA regulations.
- Check Forward Health Portal regularly for updates.
- Check emails daily and reply promptly to all communications.
- Discharge clients as per DHS guidelines 105.17(1w)(b)(f)
All. - Audit clients’ records in a timely manner.
- Graduate of a nationally accredited Nursing program.
- Current Wisconsin licensure as a Registered Nurse and minimum 1 year’s supervisory or administrative experience in personal care services or related program.
- Must pass drug test.
- Skills in leadership, organization, negotiation, conflict-resolution, and decision making.
- Effective oral, written communication and interpersonal skills.
- Basic keyboarding skills and comfort with computers.
- Ability to work well under pressure and capable of meeting deadlines.
- Ability to work independently and collaborate effectively as part of an interdisciplinary team.
- Maintain detailed work schedule on the Intranet calendar.
- Ability to maintain good working relationship with office staff as well as healthcare partners.
- Ability to work with cultural diversity and with an interpreter.
- Maintain current Wisconsin RN and driver’s licenses and reliable transportation.
The employee is regularly required to perform walking and standing activities and will need to be able to sit at a desk and complete forms on a computer screen.
