Transitional Support Program (TSP), East Windsor, CT PC#: 1212 ABOUT THE PROGRAM
TSP is a Supported Apartment Program for Young Adults ages 18-25 and is staffed 24/7 by a multidisciplinary team consisting of an Employment Specialist, Therapist, Residential Counselors, Administrative Assistant, Residential Supervisor and Program Director. TSP provides services focused on teaching daily living skills, social and recreational skills, time management skills, interpersonal skills, conflict resolution skills, coping skills and symptom management skills;
with the ultimate goal of helping Young Adults transition to independent living. TSP offers these services through both group and individual modalities.

Duties & Responsibilities Provides Direct Services and Supervision to Clients- Provides a safe, nurturing, caring atmosphere for people who reside in the programs.
- Works within program policy and guidelines to provide program services.
- Demonstrates an understanding of human development relevant to the age group, as well as demonstrates an understanding of the actual developmental levels of the clients and intervenes to promote development and appropriately adjusts expectations by considering developmental stages.
- Demonstrates an understanding of the importance of family/kinship connections for clients and supports those connections by welcoming parents, relatives, and other natural supports and facilitating planned contacts with families in the program, the community, and the family/relative home.
- Demonstrates the ability to form and maintain positive, nurturing relationships with clients by building trust and supporting competencies through interest and active engagement.
- Demonstrates the ability to teach and support the learning of a broad range of skills including those related to: relationship building, symptom management, relapse prevention, emotion regulation, age‑appropriate activities and competencies, budgeting, independent living skills. Plan and actively participate in activities to support and increase skills and competencies.
- Demonstrates an empathic understanding of complex trauma and its effects on attachment in current relationships. Incorporates this understanding in interactions with clients and responses to trauma‑based behavior.
- Demonstrates an awareness of the physical and emotional environment by contributing to the program being a place that is emotionally and physically safe, nurturing, appropriately engaging and stimulating, which fosters growth and resilience.
- Supports routines through assisting and supervising, in an age‑appropriate way, wake‑up, bedtime, personal care routines, completion of chores and laundry, meals, meal preparation, recreational activities, and homework supervision and assistance.
- Transports adult clients when necessary.
- Documents planned interventions, behavior incidents, and general observations of clients and assessment of their skills, behavior, and moods in an objective and professional manner.
- Demonstrates knowledge of, and role in, individual treatment plans, including their assessment, particular needs, behavioral goals, and identified intervention strategies.
- Maintains documentation in compliance with agency and program standards.
- Participates in and leads staff meetings, supervision, trainings and other meetings as assigned.
- Additional duties as required.
